Homecare and Rehabilitation Assistant

Our Homecare and Rehabilitation Assistant's are the foundation of our company. 

PURPOSE OF JOB:

To provide a consistent level of quality care and rehabilitation to clients, to enable them to lead as independent a lifestyle as possible, within their own home. The role is part of a team approach to promoting independence and wellbeing for all our clients. You are an advocate for your clients and as a result must be perceptive to their needs and seek help where required. The service will involve a program of personal and household tasks that are personalised for each client in the form of a care plan, drawn up by the Home care Manager, Physiotherapist and/or Occupational therapist.


DUTIES AND RESPONSIBILITIES:
1. To deliver care and other tasks as specified in the clients care plan.
2. To assist in the implementation of therapy lead treatment programs to restore, reinforce and enhance task performance, diminish or correct pathology and promote health and self sufficiency at home. The programs may include strength, range of movement and coordination exercises and activities.
3. To contribute to the care planning process by updating the home care manager of any changes in client’s needs and wishes.
4. To provide a dignified, safe and stimulating service where clients are encouraged to exercise choice and promote their independence.
5. Organise your time and resources based on the individual needs of the client.
6. To work as a team member and assist other staff as needed.
7. To be punctual and reliable.
8. To contribute to accurate, concise and timely record keeping, with proper reference to the company’s ethics and confidentiality of record keeping policy.
9. To comply with all the company policy, procedures and guidelines.
10. To comply with all responsibilities under all the relevant legislation such as manual handling and Health and Safety at Work Act.
11. Attend all mandatory training associated with your role within the company.
12. Attend all staff meetings appraisal and supervision sessions under guidance of your Home Care manager.
13. To act in a professional manner and promote the quality brand of Elderflower Homecare at all times.
14. To provide some or all of the services listed below, as per care plan within their own home.

Personal care:

  •  Assist with personal cleanliness, for example washing, bathing, showering and general personal care routine.
  • Assist with toileting needs, including catheter care and emptying commode.
  • Facilitate dressing and undressing including assistance with support stockings.
  • Help maintain safe mobility with the use of appropriate moving and handling equipment.
  • Prepare or facilitate preparation of meals.
  • Assistance with feeding.
  • Assist with medication management and compliance.

Household Tasks:

  • General cleaning and housework with the use of clients own equipment such as vacuum cleaner and dusters.
  • Carry out or assist with laundry and ironing.
  • Bed making
  • Monitoring dates in fridge and cupboards.
  • Shopping and preparation of shopping list.
  • Organisational and administrative tasks.
  • Paying bills and collecting pensions, benefits and prescriptions.

Companionship:

  • Accompany clients to appointments, shopping and social events.
  • Help keep in touch with family and friends.
  • Assist with pursuit of hobbies.
  • Assist with travel or holiday arrangements.
  • Provide general company and companionship to enhance social stimulation.

Pet Care:

  • Feed pets that live with the client.
  • Dog walking.

Senior Homecare and Rehabilitation Assistant

PURPOSE OF JOB:

This proactive role is part of a team approach to promoting independence and wellbeing for all our clients. You are an advocate for your clients and as a result must be perceptive to their needs and seek help where required. The service will involve a programme of personal and household tasks that are personalised for each client in the form of a daily routine, drawn up with the client and managers/care coordinators. As a Senior Homecare and Rehabilitation Assistant, you are a key member of the team and will be expected to daily routines and risk assessments, carry out shadow visits for new members of staff and maintain sufficient supplies of PPE and paperwork in the client files.

DUTIES AND RESPONSIBILITIES:

  1. To deliver care and other tasks as specified for individual clients.
  2. To actively contribute to the care planning process by updating shift lead of any changes in clients’ needs and wishes and to be proactive and creative in ways of engaging clients.
  3. To provide a dignified, safe and stimulating service where clients are encouraged to exercise choice and promote their independence.
  4. To organise your time and resources based on the individual needs of the client.
  5. To work as a team member and assist other staff as needed. 
  6. To be punctual and reliable.
  7. To contribute to accurate, concise and timely record keeping, both on hand-written notes and through electronic communication devices, with proper reference to the company’s ethics, confidentiality and data protection legislation and regulations.
  8. To comply with all company policies, procedures and guidelines.
  9. To comply with all responsibilities under all relevant legislation such as manual handling and Health and Safety at Work Act.
  10. To attend all mandatory training associated with your role within the company.

TASKS

Tasks to be undertaken may include:

Personal care:

Care Coordinator

PURPOSE OF JOB:

To provide and assist all staff to provide a consistent level of quality care and rehabilitation to all clients. To enable all clients to lead as independent a lifestyle as possible, within their own home.  To ensure that all client records reflect their needs and provide up-to-date information and good practice for the entire Elderflower team who carry out that care. To contribute to the out-of-hours running of the service via shift lead role. (See separate protocol for shift lead).

DUTIES AND RESPONSIBILITIES:

  1. To be the named responsible worker for an allocated number of Elderflower clients.
  2. To carry out initial care and risk assessments and put in place their initial care plan.
  3. To discuss with Service Coordinator (Sarah Saynor) the initial service requirements as requested by the client and any subsequent changes.
  4. To draw up all relevant paperwork to do with their care.  This will include care plans, daily routines, risk assessments, MAR charts, and daily record sheets.
  5. To introduce new members of staff to each client and ensure that they understand the care required and are competent to deliver that care.  This will be done through the use of shadow visits.
  6. To review the care provided and amend and update paperwork as necessary.
  7. To provide the Admin manager with all information required for People Planner and Mobizio and keep the Admin manager updated with any changes in client details.  This includes all their personal data, NOK, contingency planning and invoice details.
  8. To monitor Extra Information sheets submitted on Mobizio and record all day-to-day client issues on People Planner.  This includes updating any events, tasks, accidents and incidents and anything else that has an impact on the quality of their care.
  9. To report any concerns to the Registered Manager about clients and/or staff.  This includes any Safeguarding concerns you may have and any new information with regards to Power of Attorney, Do Not Resuscitate forms and any significant changes in clients’ health and wellbeing.
  10. To undertake regular reviews with clients to ensure that their care is fulfilling their needs and to review all paperwork.
  11. To contribute to the care planning process by updating the Registered Manager of any changes in clients’ needs and wishes.  This includes visit times and care provision.
  12. To provide a dignified, safe and stimulating service where clients are encouraged to exercise choice and promote their independence.
  13. To work as a team member and assist other staff as needed.  This will include undertaking visits as required in order to get to know all clients and to cover staff shortages.
  14. To undertake the role of shift lead on a rota basis.  To follow the shift lead protocol when undertaking this role. (See shift lead protocol)
  15. To be punctual and reliable.
  16. To contribute to accurate, concise and timely record keeping, with proper reference to the company’s ethics and confidentiality of record keeping policies and ensuring compliance with Data Protection legislation.
  17. To comply with all the company’s policies, procedures and guidelines.
  18. To comply with all responsibilities under all the relevant legislation such as Manual Handling and Health and Safety at Work Act.
  19. To attend all mandatory training associated with your role within the company.
  20. To attend all staff meetings, supervision sessions and any other meetings where your presence is requested.
