Care Coordinator
Our Homecare and Rehabilitation Assistant's are the foundation of our company.
To provide a consistent level of quality care and rehabilitation to clients, to enable them to lead as independent a lifestyle as possible, within their own home. The role is part of a team approach to promoting independence and wellbeing for all our clients. You are an advocate for your clients and as a result must be perceptive to their needs and seek help where required. The service will involve a program of personal and household tasks that are personalised for each client in the form of a care plan, drawn up by the Home care Manager, Physiotherapist and/or Occupational therapist.
DUTIES AND RESPONSIBILITIES:
1. To deliver care and other tasks as specified in the clients care plan.
2. To assist in the implementation of therapy lead treatment programs to restore, reinforce and enhance task performance, diminish or correct pathology and promote health and self sufficiency at home. The programs may include strength, range of movement and coordination exercises and activities.
3. To contribute to the care planning process by updating the home care manager of any changes in client’s needs and wishes.
4. To provide a dignified, safe and stimulating service where clients are encouraged to exercise choice and promote their independence.
5. Organise your time and resources based on the individual needs of the client.
6. To work as a team member and assist other staff as needed.
7. To be punctual and reliable.
8. To contribute to accurate, concise and timely record keeping, with proper reference to the company’s ethics and confidentiality of record keeping policy.
9. To comply with all the company policy, procedures and guidelines.
10. To comply with all responsibilities under all the relevant legislation such as manual handling and Health and Safety at Work Act.
11. Attend all mandatory training associated with your role within the company.
12. Attend all staff meetings appraisal and supervision sessions under guidance of your Home Care manager.
13. To act in a professional manner and promote the quality brand of Elderflower Homecare at all times.
14. To provide some or all of the services listed below, as per care plan within their own home.

The service will involve a programme of personal and household tasks that are personalised for each client in the form of a daily routine, drawn up with the client and managers/care coordinators. As a Senior Homecare and Rehabilitation Assistant, you are a key member of the team and will be expected to daily routines and risk assessments, carry out shadow visits for new members of staff and maintain sufficient supplies of PPE and paperwork in the client files.

To ensure that all client records reflect their needs and provide up-to-date information and good practice for the entire Elderflower team who carry out that care.
